all my fish die

good morning. I am having problem with my tank. i have 2 clown fish, and one coral beauty angel fish. they die one by one every 2 weeks. I cant figure out why, and i didnt see any white spot on them.
my water parameter
ph-8.2
ammonia -0
nitrate-0
phosphate- 0.20
i bought the salt water from local fish store

i dont have the pic of my tank now. i will take it after i back home. it is 40 gal tank and the cycled been done since January. i only have those 3 fishes and sea cucumber and some snails and hermit crabs

3 fishes in a 40 gallon should not be dying. IMO either your tank has disease, or something is wrong with the way the tank is set up, for example perhaps low oxygen or something, but a visual will help. Thanks.

What was the behavior prior to death? Lethargy, not eating, poor swimming? Reminds me of when I had brook, killed a few of my fish...

That looks like velvet to me… @vetteguy53081 and @Jay Hemdal would know better though

so the first clown has been not eating and always swim like side way for weeks. the second clown he started swiming close to the rock still eating normal. but the last fish coral beauty havent been eating much, swiming normal until yesterday he start swim close to the too of water then few hrs later he passed

Fish are hardy imo, all my fish lived through high metal environment, ich, 3.5 alk, bacterial bloom, nitrate phosphate swings dont affect them, etc

a good rule is that if they act weird, then something is wrong - I've never had velvet but I heard that one is a killer, someone mentioned start over so I think that's a valid point, no clown fish should be dying they are hardy little fellas

if you don't have too much left in your tank I would do a quick triton icp test (cuz its fastest), or just buy some tropic marin pro salt (or instant ocean etc) and RODI water from RELIABLE LFS and mix yourself.

Given the time frame, dying over weeks, the only parasite that would account for that are flukes, either skin or gill flukes.

The fish in pic is hard to make a determination. Dont wait in the future for fish to die one by one.
Any signs youve noticed like loss of color, heavy breathing. loss of appetite, scratching, gasping at surface as examples?
I suspect flukes but signs as mention will help with assessment.
Have you tested the LFS water to assure no issues? May not be a water issue, but I ask to eliminate possibilities.
How are you testing the water?
