Any new fish I add dies with in a week. Original fish in affected???

Hoping some may have experience this or have some clue of a direction to go in.

I added a flame angel 6 days ago, first 3 days eating like a pig not getting bullied etc. day 4 I notice it was hiding not keen on eating so I started directing food into where ever it was hiding and it would grab a couple bits and that was it. Day 6 dead.

i couldn’t see any signs of ich or increased respiratory rate. No bullying

Fresh water dipped the dead flame and nothing exited the skin or gills.

now this has happen with my flame hawk and a potters angel as well.

my purple tang lawn mower blenny and malunurus wrasse have been unaffected from this event of the 3 fish dying over the last couple of months.

A couple possibilities-
Acclimation: How are you acclimating and for how long ?

FW dip - is there a reason you performed this step? FW dip can stress a fish within those short 5 mins

With no rapid breathing or other tell-tale signs, can be :

- addition of chemicals
- Oxygen depletion
- False test readings
- high temperature and salinity
- stray voltage
- use of tap water
- Cleaning products used near the tank

** Any pets in house that may be urination into sump water ?

How big of a system do you have ? How long has the tank been set up and running ?

Sorry about the typo in the title

- I fresh water dipped the fish AFTER it died to see if anything would bail off.

- salinity is 1.026 0 phos 1 nitrate 0 ammonia

- I drip acclimated it over a corse of 2 hours playing it safe being a $500 fish here in Australia.

- RO water used

- no chemicals used near tank and my dog hasn’t ticked in the sump as It’s a closed cab and I would of notice it

- no stray voltage I can feel

- temp 24C (not sure what that is in F)

- tank is about a year old mixed reef.

I haven’t had any bacteria blooms i don’t dose carbon source or anything other than randys, no cyano or any other issues I can see.

I find it odd that all other fish have been un affected people over the few months this has happen when adding anyone new.

One thing I didn’t check was the salinity the fish came from my self but I did ask and was assured they were in 1.024
