Not sure how well you can tell in the pic but the head on the left keeps closing up on me. It is open sometimes but it has been doing this.

The baby heads look fine and the big one on the right looks fine. Is it a feeding response or what?

It doesn't look too bad. I have one like this and it keeps sprouting heads ever sense I added a T5 light. That head may not be getting enough light. Are you using LEDs? Mine is in the middle of my tank about 20 inches from the light.
 
It should be. This was it a couple days ago.

They inflate and deflate time to time. Doesn't look like anything to worry about.
It opened back up this afternoon. I worry about everything.
