I ask this because I know some anemones can release toxins into the water when they die please let me know what I should do here and yes I am sure its dying I've had one die on me before look exactly like this one and behaved just as this one is... barely attatched and all tentacles deflated trying to completely close up (but cant fully because it's so big which is why I am wondering if its death will be toxic) and its mouth is open with what looks to be his insides coming out....

Need pics. Too many people give up on nems too quickly because they get scared.

What size tank do you have? Have your treated it with cipro? Looks like it could use a full treatment and recover.

Stressed but not dead. Place it in a plastic container within tank and keep and eye on it. Salinity range 1.024-1.026 with moderate light and water flow and temp range of 77-79F

LTA needs to be on a sandbed, not rock. How deep is your sandbed? Minimum of 2" and that's pushing it. It's crazy to me that you would look at the nem in that pic and think it should be tossed. Think about rehoming it. If you keep it, give it a proper environment then reevaluate. I would not isolate it at this point, you will just stress it out further.
