Black ich is easily treatable with praziquantel (PraziPro,) which is reef safe.

The fox face’s tail looks pretty bad. Is it getting bullied? Looks like a bacterial infection.

Nope
That’s a rock
The tail is fine and totally in tact
Though it is sometimes chased by the maroon clown

Close the skimmer and remove the carbon for 24 hours. That single dose should eliminate it, but just to be sure, repeat the dose after a week.

Sadly it has spread to the flame angel and it does not look good. Started polyplab last night. The foxface is now mostly spot free and looks fine. Worried about the angel though.

What does it look like? Is it still black spots?

Polyplab Medic is peroxide salt based and ineffective unless early stage issue at times.
Do you have a pic of angel as of today?

A little over a year ago something killed my tang, butterfly, and 2 angelfish. It also got my foxface a little sick. Left everything else seemingly unaffected. I did a copper QT but I think I failed to kill it.

Flame angel died
Lyretail hogfish jumped seemingly unrelated
Marroon clown fish looks rough
Do you have any corals or inverts in the tank?

Are you sure there aren’t any spots? Sometimes the spots go, but the ich or velvet temporarily fell off to reproduce and build larger numbers to reinfect them.

GSP and mushrooms. I've tried in tank copper before, and it obviously failed in the long run

Its really hard to see the clown as its hiding but it looks like velvet or uronema which is completely different to the large spots that were on the fox.

I'm just confused what started it. Nothing has been added in forever and tank conditions have been constant

Lets just think about if ich and velvet can be dormant. A single fish fight could cause stress enough for the villian to get a foot hold. Hypothetically speaking here.

My fish have been with me cbb and scopas 2021 or 22 of september, and sometimes they just brawl with each other.

I never treated for ich. But, i ran very dense filter socks when ever i added new fish for a month. 15 or 25microns.

Actually I take that back as I also have the falco
Both pretty fish but absolute pricks

I never could find the jerk my husband picked out at the fish store. he was one of those hawk fish. Forget which one. But he was an butt the moment he was let go into the tank. INSTANT BULLY. Faster than a cup of ramen.
