Hammer and torch polyp bail out?

Tank has been doing great for the last couple months. Yesterday I lost 2 heads of golden hammer, and it looks like 2 heads of torch are not far behind. They looked totally normal ~3 days ago, a little closed up 2 days ago, and completely out of the skeleton last night.

Changes in the last month: Added BRS carbon reactor, using red sea carbon to help with the tank smell. Ran 1/2 cup and had the flow restrictor dialed in at half.
Received a coral shipment from AquaSD. Dipped everything in coral revive. Had a hammer melt within 1 day of receiving. No BJD, and no BJD on the heads that are disintegrating.

Tested everything a week ago at the following parameters -
Alk 10.1dkh (dialed dosing back)
Cal at 460ppm
N03 at ~4ppm
p04 at .04-06 ppm
Salinity 35.1 ppt
Temp 78.5-79
Ph 8.1-8.38

Did ~15 gallon WC last night, and will do another 15 tonight. Hope to have time to test everything before I do the WC.

These corals have been through some crazy things over the years. Very surprised and concerned that there is an issue now.. Nearly every other coral in the tank is looking great. Zoas, Rics, Nems, RFA's, Lobo, Cynerina, chalices, shrooms.. My 2 other hammer's are also looking a bit shrunken in and closed up, but no loss on them as of yet. All spread out through the tank.

Your tank shouldn't smell. What does it smell like?
Just smells like a reef tank.. Water, algae, skimmer.. the usual. Its an all in one, so most of the smelly stuff is right in the open instead of in a stand. It isnt something you smell through the whole house or pungent, Just when you are examining the tank or doing maintenance you can smell it.
