Hammer bail out

Can anyone see a reason why this hammer has started to bail out? Seemed to be fine the past few months, looks like maybe some vermetid snails may be attached to the base of the coral, not sure if this would all of a sudden cause the issue.

Please post your parameters and anything you are doing differently. A complete shot of the tank would help along with your filtration.
I just took over taking care of this tank, parameters are as follows:
Salinity: 35PPT
ALK: 9.0
Cal: 450
Mag: 1350
Will report back on nitrates and phosphates

All other hammers, acros, montis, and scullys seem to be doing fine. I can post more pictures in a bit

Phosphates?
Flow? is this hammer in the same spot. Different spot too high flow?
Light in this area. Too high? It does look like it's high in the aquascape. It may be possible the vermited snail causing irritation. Remove them now while you dont have too many.
