Hawaiian Cleaner Wrasse Constipation

I am posting for help with my Hawaiian cleaner wrasse

Obtained 2 months ago from Biota in juvenile coloration (black and purple, no yellow). Has subsequently grown and gained more adult coloration as seen in the photo. Last week (4/8) I noticed he had a huge belly after eating many newly hatched (less than 6 hours hatched) baby brine shrimp that I was target feeding to my mandarin. He had been readily eating frozen mysis, nori and the occasional small bit of flake. I assumed he had overeaten and that the large belly would go down over the next couple days (and it did) but the following day he would not eat anything but live baby brine. Because several fish in the tank had begun flashing during the week, including the wrasse, I treated the tank with Prazipro on 4/9, wondering if flukes were present. I had previously treated with two doses of prazipro approximately the same time when I received the wrasse (late February) due to concern my anthias had flukes. Anthias stopped flashing and I had assumed the problem was resolved at that time.

Over the next several days after prazi treatment the wrasse regained an appetite and was back to eating bits of mysis and flake this past weekend and until 4/15. Yesterday, 4/16, he stopped eating mysis though was still eating live baby brine. He seemed less inquisitive than the day prior. As it had been a week since my prazipro dose, I dosed for the second time (fourth if you count 2 months prior) last night (4/16).

Today, I noticed a small outpouching in his anal area, presumably early prolapse. He is still eating live baby brine and making an effort to clean fish. He is not paying attention to frozen mysis or nori. He had been back to acting normally, as best I could tell, 2 days ago. I am concerned for a number of possibilities - internal parasites, constipation from over eating, or even constipation from perhaps ingesting an unhatched brine shrimp egg (a few have gotten into the tank at times). Certainly, he could still be dealing with flukes or even ich (though no spots are seen on any of the fish). I was hoping to get advice for what to do next, if anything. I have attached a photo from a few minutes ago. You can see the area beneath his belly. The spot that appears to be on his tail is actually on the glass.

1. Brine shrimp tends to reduce constipation
2. Prazipro may be degraded if not dosed in a hospital tank - so its hard to know if this is fluke related - the symptoms you describe do not suggest this.
3. it sounds like multiple of your fish have some illness that might be flukes - with the flashing - but can also be other things - in any case - if its not ich, all should be removed to a 'hospital tank' and treated - with your tank being left fallow (based on the information you provided) - If you can give any more history, etc - it might be helpful. Do you have a picture of the whole tank - and do you have a video of the flashing, etc?

4. Your picture to me does not show a clear prolapse

Continue eating brine shrimp but reduce amount fed over 24 hours to see if swelling is reduced. Brine eggs can and would cause small blockage and these fish as with mine have very small stomachs
The extension which I see is not abnormal when full.
