Imhave this guy for about 2 years and in the past weeks he hasn't been well, but recently he stopped eating.

There are no exterior signs.

He seems to have trouble keeping his balance, but he's very skittish, it's impossible to catch him.

When it started I had a moderate dino outbreak and I thought maybe he was not feeling well from eating it.

The dinos are gone (I balanced the nutrients) and now there's just a little bit of cyano.

He looks stiff, like he's in pain. He looks a bit fat. I have flame angel that is very fat. Maybe intestinal problems?

I feed mostly frozen mysis and a little bit of a mix of flakes throughout the day.

Can you post pics of the body of the fish? Clear ones?

That fish looks unhealthy, I agree. Stressed and weak. This is surprising, given his girth. Guessing you’re dealing with a parasite (inside or out) or an infection (internal or external perhaps).

Thanks for the help.

It's a difficult fish to take a photo.

He does look fat and he hasn't been eating for a few days.

He has ich?

I see some spots that seem to show up in all of the photos (not the two under/in front of the eye, those are normal). I wonder if they're flukes. I'd first try a freshwater dip in a dark bucket. if you see "sesame seed" looking things drop off during the 5 minute dip, you've got flukes.

The others could be ich but I'd be surprised if ich at that level had that much of a tole on a fish-- although repeated exposure over months could do it, as ich often harms the gills much more and much sooner than the exterior of the fish shows "spots". Gills have little to no protection from the parasite, so it's a strong target.

Is he breathing heavily? Hard to tell from the photo.

His mouth is too open too.
That's a good observation I somehow overlooked. I don't see evidence of uronema otherwise and it's unlikely to suddenly hit a fish after being established fat and healthy in a display tank. Could be a locked jaw (injury) that has swollen. For that, two doses of epsom salt (pure, organic -- not scented) 1 Tablespoon per 5 gallons a couple of days apart may help with that. Since epsom salt is magnesium, this is a reef safe remedy.

Thanks.

The mouth looks stiff and open but not breathing heavily.

I would have to tear the whole tank down to catch him.

I have praziquantel (a 100ml bottle of Sera Tremazol with 7.37 grams).

The tank volume is about 200 gallons).

I can't catch him.

I had a bad batch of salt from aqua forest that made tank magnesium go to 1650.

If he has ich than it's more likely to be opportunistic and not the main source of the problem.

You can treat with prazi and see what happens -- dose it 5-7 days apart. It's "reef safe" although it can anger inverts. Coral won't be happy for a day or two after, and your skimmer will go nuts. I've used it in my DT's many times, however.

I've had to remove several fish by disassembling the tank, I know it's not fun.

Thanks.

So, for 200 gallons (750 litres) gallons it should be 750 x 2 mg = 1500 mg of praziquantel?

The bottle has instructions to dose 1ml/15 litres (4 gallons) for gill parasites and 1ml/20ml for other parasites.

The bottle has 100ml with 7.47gr of praziquantel.

1ml should have 0.074gr of prazi (74 miligrams).

So they advise 74miligrams per 4 gallons ( 18.5 miligrams / gallon).

This doesn't seem right.
