Looks like he lost.
Look like mouth marks.
Anybody you have that would challenge him?
Me, I’d do nothing right yet, he’s got some scrapes.
He can easily regenerate that in a month.

You can use this hashtag #fishmedic to get help. It looks like an injury - from what - unknown. If its worsening - I would place it in a quarantine tank with an antibiotic. More information would be helpful. See the link in my signature for help answering some questions that can help us help you - and the rest of your fish

Evening,

Sorry about the trouble. Just to be sure have you run thru your basic water chemistry numbers, Also who are the tank mates and how long have you had him.

Hi I recently added a gramma loreto to my tank it is very active and seems to pick off whatever is floating in the water column and the small critters on the glass it is the only fish in my 15 gallon tank and I have purchased it at a local petco. When I feed the fish it responds well and seems to be healthy my tank is about 3 1/2 years old and being a mature system may help sorry to hear that your fish is not doing well I know I have lost some myself and I have always felt bad about losing life in my system and trust me I am more tolerant of the life that shows up in the system than most people and maybe that is why I do so well with a very simple system.

That's a very nice gramma, but I don't know that we needed reference pictures for what they're supposed to look like.
