You Tang is getting the crap kicked out of him by your others. This looks like injuries due to aggression and he has a split fin that wasn't there the other day.

Some be of those spots might be ich, but the clear ones are mucus plugs, they are more attached at the base than ich would be. Still, ich and mucus plugs are commonly seen together on tangs.

Best to setup a QT for all the inhabitants at this point then right?

Yeah I didn’t notice it this morning. What are some options I can take in your opinion

You have a Powder Blue and a Gem so more than likely one of them is slashing your Tomini. I have three large Tangs and usually when they go at is at night, so you don't see the damage until the next morning. The cause at least for mine is squabbling over sleeping spots. A new Tang will pick out a spot in a Rock Crevasse and usually it is already claimed by one of the established ones, so a fight occurs. My 3 each have a spot they go to every night so you can see how a new one could cause problems. You could try making a new spot or two that may help.
Post another Video with a broadside shot and let's have Jay take a look. After all, he is the Doctor.

No, it won't hurt. Some of your corals may close up for a bit. Here is info about using prazi:

Your Tank looks good, however you need more hiding spots for your Tangs. My Hippo will wedge himself into nooks and crannies that make you wonder how. My others just like a small cave to sleep in.

Thank you both @W31Olds @Jay Hemdal for your advise. I’m only 3years into this hobby and fortunately (until now) haven’t had a run in with any diseases. Will monitor the guy for another day or so and then make a call on medicating them in the tank or out. Thanks again.

For anyone that’s curious or reading this thread- it was in fact flukes. Took the guy out as it worsened and did a freshwater dip. Saw these oval shaped, sesame sized thingies fall off of the tomini. Fella is in my sump now so I don’t have to try and catch em thru the rock work to check on em. Started dosing prazi yesterday once it came in the mail. Other fish not showing.

Those are Neobenedenia flukes. They can be difficult to treat. You'll need to dose the prazi 3x, about 7 days apart.
