Is my anemone sick?

My bubble tip was very happy for over a year. All of a sudden it disappeared and then was shrunken, only a quarter of what it was, devoid of color and just looking bad. he fell off the rock he was on but is now attached to a small rock on the floor. He has a couple of tentacles floating around, didn’t seem squishy when touched to try to move him. So scared he is not going to make it. the first 3 pictures are him in his glory days. I wanted to post more pictures, but can not get them to attach. He is now the size of a quarter, and white. On the floor of the aquarium. is there anything I can do to save him at this point? Thanks

Today he puffed up on the bottom, attach himself to the small rock again. We moved him to a higher point in the tank. He moved off the rock and into a crack. we also fed him, he appearEd to eat.
 
Clearly it is not happy. Has anything changed in your tank? What are your parameters?
