Also monitor Ammonia levels while in quarantine with a reliable test kit and add aeration during treatment using an air stone.

It does not work and you will waste valuable time with proper treatment. You can get a used tank from thrift store or marketplace and even use a rubbermaid type tubThank you.
Unfortunately don’t have a tank large enough for all the fish in this one and that tank also has fish in it currently.
I already had a new unopened bottle of medic, was thinking to use that, I have already desolved the recommended dose in a beaker, just not added yet.
If you’re talking about Polyp Lab reef medic, it only works when ich is mild. Like when you see 3-4 spots on the fish. In your case it’s way too advanced. This fish is infested. Hypo is not a good option at this time and there is coral right within the pics. OP said another tank was not an options and suggestions were offered of r used tanks or a tubYou have a couple of options but may not be optimal.
1. If you have no inverts and this is a FOWLR you can run Copperpower in the Tank. It sounds like you have them though.
2. If you have inverts and corals maybe remove them and run Hyposalinity.
3. Could you put a divider in your largest Tank, place the Corals and other Inverts in the other Tank, and then run Hypo on the Large Tank?
One final note. You said you have a second Tank, so Biosecurity is very important, but there is a chance you have already introduced Ich in that one also.
I agree that his fish is infested and Hospital Tanks/ Tubs are best option but do take time to aquire and get setup.This fish is infested. Yes, that’s a severe case of ich. Are any other fish showing spots today?

I think your next best option would be hyposalinity, polyp lab medic rarely works and pretty much fails completely for severe cases like this.

Don’t that usually take 3-5 days, only noticed spots last night, 100% did not have them in the afternoon.
