Lta injury or parasite maybe?

My lta was detached and floating around got him put back in his bull pin trying to get reattached I took pictures I've had him for a month now had a hard time anchoring when I purchased him parameters are -
Cal - 560 ppm
Mag - 1,550
Phos - .15 ppm
NO2 - 0 ppm
NO3 - 0 ppm
NH3-4 - 0 ppm
pH - 8.2
Sal - 35 ppm
Temp 80°f
He is still taking food just recently ate a fingernail size chunk of shrimp.
