Need help with my clove polyp

Hello,

I have got this clove polyp for 6 months now and it was doing great until I moved it a little top on my tank. Then came a hair algae outbreak but thankfully controling the light it washed away. The clove polyp and some of my zoas are started to degrade my Zoas are melting away and polyps from the Clove polyp are melting as well. A couple head started to apear else where in the back and a tiny frag was latched to the rock after I moved it. So I sort of have two frags out of the main colony dling okey but the main colony seems to be sick. Any help and suggestion would be great my water parameters are as follows:
Kh 9-11
Ph 8.2
Mag 1280
Calcium 380-400
Nitrate undetectable
Phosphate not detectable ammonia is zero

I think the nitrates and phos being so low could be contributing to the health of the colonies. They like slightly dirty water.
