What is the best phosphate control media that can be put in a bag and placed in hang on back aqua clear?

Kent Phosphate Sponge hands down. Been using it myself on and off in a small HOB filter for the last year. Great product IMO.

You could even try getting a GFO reactor. With this you can mount it near the tank with the powerhead just inside. I personally have a sump so my reactor is down inside that but it has worked wonders for my phosphate issues.
 
Any GFO media in a small bag will work. Test the phosphates first though to make sure you even need it before investing in media of any sort.
