Possible HITH on BiColor Blenny?

Unfortunately my bi color blenny (got him Nov 2) has a literal HOLE in his forehead, and abrasions on his face. Took him back to the reef store, and decided not to swap him because I love his grumpy face. They confirmed, yes, there is a small chunk of flesh missing from in between his eyes. They put a blue antiseptic (I think) in the water I brought him in, and also gave me some extra with these instructions: ATTEMPT to catch the blenny, and then with a qtip, dab some extra antiseptic on it. I purchased seachem’s Kanaplex and Focus powder to add to the spiralina brine shrimp I feed him. He soaked in the antiseptic for 30mins before I return him to the tank. I’m worried it’s HITH but it seems kinda unlikely since he’s still fairly active and eating. However, it is quite literally… a hole in his noggin. Perhaps, he was injured by another fish in the store? Cause looking back at photos, he had the hole when I purchased him. Should I go as far as to quarantine him? Any other suggestions? The second clip is to show that the white of the hole/abrasion/idfk does not go away with color change. Thanks for bearing with a new hobbyist like myself

Quarantine may be a start if treatment is warranted but personally if it had this hole, I would return fish for a healthy one as I cant see why they sold it like this and did not see it themselves. If youre going to nurse it, they can at least furnish needed meds but first, Please post pics and even video using white light intensity no blue to see what you are describing

I don’t recall seeing HLLE in blennies before.

Don’t mix kanaplex and focus - it is toxic if you get the dose wrong, and unless you calculate carefully, you won’t know the dose. The dose is 23 mg per pound of fish weight, so almost impossible to do correctly.
