So he's finally eating and getting a tad less stressed. It's been 40 days in QT. Still don't see him much in QT. Feeding live blackworms. When I transfer to my DT will he just eat off the rocks? Should I use an acclimation box first? Or would that just freak him out even more? This one is crazy scare.

Whether you should use a social acclimation box would depend on what else is in the tank already.
The Potter's should pick off the rocks - but what's on them? Is there enough algae, sponge, 'pods or other food to make picking profitable?
He may disappear for a while, but hopefully he'll come back out to play.

I think safe enough to put blackworms in ... but the Potter's will have to move quickly! Those other fish will enjoy the worms as well.

The flame hawk is the only potential I really see for trouble. You can use the box if you'd like (it's rarely a horrible idea), but I would expect the angel to be able to roam the display relatively quickly.

What size DT?
I added my Potters into my 240g that had a Flame Hawk and three Tangs and a Magnificent Foxface. Never had a problem.

I would say use it....let everyone get used to each other and then let him roam....never a bad idea IMO....and blackworms in your DT are fine....I use them all the time.

I agree, the flame hawk may be trouble and become aggressive towards the potters. Had a falco hawk once years ago and added a coral beauty months after the hawk. The hawk killed the coral beauty before I could catch it. Flame may not be as bad, but they can become territorial once established. Roll of the dice. A 60 gallon isn't a very large foot print for territory.
With that said, a Socialization box isn't a bad idea.
By the way, if I didn't have SPS in my current tank I would have a potters in a heartbeat. My favorite centropyge!
I would more concerned about him taking to other foods and grazing once he gets into the DT. QT not only serves as a protection against disease entering your DT, but also serves to get a fish accustomed to living in captivity. If all he's been eating for 40 days is live black worms, it's 50/50 if he'll take to grazing and other foods.
