Sick Fish,DISEASE unknown to me

Hello to everyone,

(short intro) I have started my Reef Aquarium in Avgust last Year so cca 5 Months ago,added Corals,2x Clowns and 1x Blenny Bicolor.
They have been eating and swiming just fine except that Bleny was scratching himself of the Sand a few Times a Day until.

I have added 1x Gramma Loreto and 1x Firefish on 2.1.2026.
Firefish was doing fine and eating since the first Day while Gramma got white dots,just a few of them and she was eating also.
Gramma also started scratching himself frequently and swimming with closed Fins.

I was doing normal Waterchanges and on 12.1.2026 i forgot to power on my Heater for 1-2 Hours,temperature dropped 2° C from 25.5 to cca 23 ° and i forgot to turn off my Skimmer (i was a little bit in a rush) and he overflowed whole Cup in to the Aquarium.
So i turned on my Heater and Skimmer also and everything was fine,corals,snails,fishes nothing Died.

On 13.1.2026 Clownfish got ich a lot of it,Gramma also and both started swimming with closed Fins.

Today 14.1.2026 Firefish was dead (Clown and Gramma) bullied her a bit but i hope that is the reason.
Clownfish is doing better today,started eating again,Gramma is eating also but all three of them scratch themselfes especially after they eat.

I have 1x Cleaner Shrimp,4xSnails and Corals(Sps,Lps and softies) in the Tank also.

Beside Skimmer i have Sicce 13w UV-Lamp (bought it 3 Days before and it is running 24h) and Chaeto Algae in the Sump.

I don't know what to treat them with and how?

Sounds like flukes which UV would not help with. Can you post a Video using no blue lights but rather bright white light intensity?

I would treat with prazi pro in tank if it’s something like flukes do you see anything hanging off them looking kind of like little worms? reef medic works well ime for ich unless you can quarantine all the fish and treat with copper
Stressors can cause fish to get sick with something they were previously able to fight off

It sounds more like end stage ich to me, you mentioned the fish had white dots.

Can you post a video of the remaining fish under white light?

Once fish loss from a disease has begun, it can be difficult to stop it in time to keep more fish from dying.

Those are 3 Videos from Today,they all look better except that they scratch themselfes still as you can see on the Videos.All three of them eat like crazy.
I have talked to one of my friends who have had similar type of disease,same Fish Gramma and Blenny and he was having success with Praziquantel.

So i have ordered a Tremazol online and please tell me if i should dose it or not? Maybe something else,some other medication and if what should be a dosage,should i change Water more than usual,Skimmer on,UV Lamp on?

I can’t rule out ich from this video. It doesn’t look like flukes. It could possibly be brooklynella, but ich is more likely based on how the fish is swimming.

Flukes can be treated in a display using praziquantel, but generally, ich cannot be as the two most effective treatments (copper or hyposalinity) will kill any corals or invertebrates.

It looks and sounds like Ich - especially without QT. What would be the other options? 1. Brooklynella - unlikely with the pattern you're showing. 2. Velvet - not the presentation at all. All of your fish should be treated in a copper protocol. and separate from the main tank. The main tank should be left fishes for 6-8 weeks. Whenever I go through this type of problem - I have done it longer than 8 weeks
