How long has the tank been running and have you ever change water before?
Yeah. Been having the tank for a year and I do water changes monthly, unless the water quality looks very bad. It's worked for me before but right now I think that the filter media needs to be changed :( So many incidents happening

For the time being, filter media can be rinsed under the sink to get the dirt off.
Even after changing filters you still want to do very frequent water changes.

Hey guys! I wanted to tell you that Gem feels better :D I did a 50% water change yesterday and she healed almost right away. Thank you for helping me. I was thinking of not telling because everytime I tell something the opposite happens XD but I’m very happy that you guys helped me, and she’s very happy too! May your reefs and aquariums flourish, and your fish rejoice their whole life :)
