A couple things...when you add the picture.
How long have you had him?
If he is new, has he been in quarantine? -- no judgement if he hasn't so honesty is best. But if he has, has he been treated with any sort of medication so far?

3 months. Eating until now. I did not at. Have had really bad luck with it in the past. No meds yet.

I have him separated now but inthe same tank

The disease experts will definitely need a better photo. All I can see is one original photo of a half-covered tang. I know R2R supporters can edit their posts, but I see that you are not one. I'm not certain you can edit photos. So you will probably have to place a new photo onto a new post on this thread.

Meanwhile, I'm getting a larger container going, but will still be separated in my main tank. I can do something else if needed. For now:
Last I checked:
Salst: 1.035
ammonia: 0
nitrite: 0
Silicate: looked like 0, but not sure
Phosphate: looked like <3, but not sure
nitrate: would have to check
PH: about 8
ORP (meaningless? but I have the probe ...): 266. stays between 250 and 270
temp: 76. swung up to 78 and back down over the last week

He is now in my sump in a plastic container. I have verified that it has flow through it by pitting in a flake and watching it swirl. Also added some macro he gas eaten in the past.

Will wait for advice.

I can't offer much advice accept is that your salinity that's 1.035? If so that's way too high. I'd slowly lower it to 1.026. Also I believe that keeping it in a small container will stress it out and make the sickness worse.

Do you have a QT available? There is most definitely infection there, probably secondary to flukes and what might look like Black Ich as well? The black marks could be part of the infection, too.

I’d do a freshwater dip to check for flukes, followed by a bath in acriflavine or methylene blue (antiseptic)and QT and antibiotics. Maybe the trifecta for 10 days (metroplex, Kanaplex, Furan-2), and if the freshwater dip shows flukes - General cure in the place of two metroplex doses, 5-7 days apart.

I can set up a hospital tank. Have to check the size. Will I have to sacrafice live rock to this? can drop in some rubble. don't want him going through a cycle.

Not sure how to do all the items you listed, but google is my friend. Will report back. I have metroplex and canaplex in the house, not sure about furan 2 but can check.

I agree. That’s an emaciated fish as well. Often velvet will leave fish this way towards the end as well. Sorry :(
