Unfortunately, its sounds like a condition often seen with them known as UNWND. Many of these type of wrasses courtesy Of Jay encounter UNWD which shows neurological symptoms where it either cannot swim well, swims tail down or swims with a bent spine and despite all this, the wrasse will still attempt to feed and display no damage such as crashing into objects. Other could be depending where fish is from, cyanide capture which is delayed but the fish ends up in this state
I just lost the same wrasse this week with identical symptoms and situation. Sad and disappointing.

yeah it was out of qt. I think it was scared so it hid in the sand for 3 whole months, and I definitely thought that the fish was long gone. It came out of the sand swimming weirdly. However, today it swims normally with all the other fish and grazes the rocks. It seems to only come out near 5 pm and goes to sleep at lights out (10pm), do you know if thats normal? He is eating and is all fine right now though.Can you better clarify the timeline? You said it was hiding in the sand for 3 months? That was after quarantine? How long was the quarantine and what did it consist of?
That amount of time under the sand would cause severe malnutrition in the fish, and it may be weak from that.
