Torch Losing Flesh

I got this torch from the coral swap in mid October, it was losing the flesh and it's been in my tank since then, I see is puffy and everything but I see part of it the skeleton is exposed.
1) Is there something I could do to help the torch to be fully healed?
I am wondering what I should do since I never experienced this.

2) Also do I need to put in in a lower flow area?

3) Why it is detaching from the skeleton?

4) Do I have to give her any medication or dipping?

Sorry to tell you but that torch is on its final journey.

How old is your tank?
Did you dip it before adding it to your tank?
First thing to check is all your water parameters, after that you check flow & lighting.

If those are all in check, it's just the classic euphylia "disease", i had some dying on me without any reason before aswell.
 
The flesh band that attaches it to the skeleton is completely receded and the torch is slowly detaching and will perish. I don’t see signs of brown jelly disease, but it’s hard to tell from the photos, so this is likely a flow and nutrition situation. Lower flow and target feeding are suggested. Along with hydrogen peroxide dip to clean that exposed skeleton.
