We had 2 orange clown fish but the male passed and we added 2 black ones. Everything seemed to be going fine and everyone was getting along. Now the black ones seem to have split. Not sure if this guy is getting picked on and stressed out or if he has a disease. All the other fish have no signs of anything and look great. He has been in the DT for a month and was in QT a month before that.

Welcome to Reef2Reef!
I see two issues at the same time; probable tank mate aggression and a disease know as Brooklynella (that is causing the white mucus on its head).
Can you move the clowns back to the quarantine tank and do you think you can get medication with formalin in it? You can give them a 5 minute FW dip when moving them to the QT, that can buy you some time.

He passed shortly after moving him to QT. This tank is approximately 8 years old and was given to me by a friend who could no longer take care of it. I have had it for 6 months and believe it has ich which and I guess brooklynella it seems. I’m thinking of starting a new system setting up treatment tanks and transferring the fish to the new tank once established so I can leave this tank fallow for two months to starve all parasites. ‍I have done pretty good for a new tank owner at keeping the parameters but I am learning about all these unseen diseases as I go.

No surprise 3 clowns didn't work out. Rehome the odd one out.

So sorry for your loss. It's definitely brook. You need to treat the other clowns as well, or else they'll die quickly as well.
Some info about brook:
