I currently have a regal angelfish in QT that is showing similar clinical signs. It is in 2.4ppm Copper power and did the first dose of fluke solve (prazi uk version) yesterday. Planning a water change tomorrow then 2nd prazi dose on day 4 then another water change 48hrs later then a 3rd and final prazi dose on day 8.

I have just ordered Kanaplax from the US to have on standby incase the situation deteriorates. I also now have agar agar to bind the prazi with food to treat intestinal parasites.

I can see @Jay Hemdal that you have mentioned poor water quality can cause these. So could it just be the fact that this fish is in a copper/prazi treatment that is causing these? Once 14 days is complete, they will be moved to a sterile observation tank for 2 weeks prior to introducing to my display. Hoping im taking the right course of action here.

Can you post a clear picture of the fish taken under white lights? I need to see what you are describing.

As a side note; your prazi spacing is too close together. That works for live bearing flukes, but will fail if you have an egg laying species. The timing for egg layers also works for live bearers, so it is best to go with that. Here is an outline of dosing timing:
