Are these fish going back to the DT after treatment +fallow period or are they new fish going in to a DT with established fish?

If there are no fish in the tank I would match temp and salinity and introduce. If there are fish in the tank, match temp/salinity and place them in an acclimation box for a couple of days.

What @saltyhog said is correct. Did you follow a full fallow period? If not, I would recommend leaving your tank without fish for a full 76 days since the last fish was removed from the tank. This will (outside rare occasions) kill the ich life cycle completely in your DT.

Freshwater filter only or is it? Have you ever used an HOB filter on a saltwater tank?
