I don't know what this is or how to treat it. Any help is welcome.

It’s a long shot but maybe Popeye or some type of bacterial infection.

If I had to take a guess... the fairly slow progression makes me think this is not bacterial but either parasitic or fungal. This is just a guess...

Whatever it is, even if you can cure the infection, chances are the scar tissue will leave him blind in that eye. How is the other eye?

your fish looks very thin. Throw a lot of food that will entice him . Nori, brine, pellets, whatever , but he needs to it.

Ah, just seeing this post. The furan is a good broad spectrum antibiotic, but obviously not for this particular issue. That is a very severe infection. When it becomes deep enough, the brain may become involved. That, combined with how thin it is would make me consider euthanasia .

Hey I just wanted to say I appreciated your response and that is indeed what I ended up doing before it suffered any longer.

I stumbled across reef2reef initially looking for help. I didn’t add it to my phone as an app so I never received notifications but I recently came back to the forum and now it’s an app on my phone with notifications. So again thank you.
