I lost three fish in QT last night.

How do I diagnose the problem. A fourth made it.

I put them in a small container with fresh water to see if they had flukes. After that?

I've had the. 18 days in QT.

We will need more info than that to help. What kind of QT are you running? What is your set up like? Do you have an ammonia badge? Did you test for ammonia? What kinds and sizes of fish did you have together?

Sorry.

Hospital style QT 20 tall
Pvc
Air stone
Power head
Heater
Tray with sand.
Ammonia badge.

5 gallon water changes every three days. Prime in between.
Two rounds of prazipro.

I had Tierra anthias with a leopard wrasse. I lost all except a male anthias. It's been 19 days. Last night all anthias ate. Leopard has been nipping but acting reclusive and only comes out a few times a day.

My ammonia badge doesn't help me much because of the prime.
I did test for nitrite two days ago and it was 0ppm.

I'm slightly color blind on this badge because of a yellow light in the room. Usually between yellow and gray.

OK. so to help with the ammonia badge you'll want to use a white light above or behind it. You'll be able to make out the color much better that way. I don't recommend mixing prime with any medication. Some people get away with combining it with things like prazi or antibiotics, but not everybody.

Were the fish displaying any symptoms? heavy breathing? Flashing/scratching? spots? Red marks?

No flashing. Breathing was eradic on the wrasse. One of the anthias became reclusive yesterday morning but was out when I came home.
The third anthias is a complete surprise. I think the possibility that ammonia got them would be highest on my list.
But there was no flashing, scratching, or hiding by the anthias until yesterday.

Following along as had a horrible run with my qt and had to empty and disinfect. Best of luck with this! Melypr1985 and Humblefish, among others are beyond helpful and know more then anyone I have come across, so you are in the right place.

Ammonia is first on my list here. Followed by the possibility of an infection or uronema. I have a difficult time with both infections and uronema with anthias at the store. They come in with these things and start dying after a week of being (i thought) fine.

A gram negative infection can kill as quickly as velvet and with less signs. Most times you'll see (in both cases) red marks on the fish but about half the time it's right at the end. I'm not saying that's what happened, just a possibility. I still think ammonia or the combo of prime + prazi was a contributing factor.

Combining an ammonia reducer, such as Prime, with Prazipro will sometimes lead to fish deaths.

Also, did you notice any behavioral symptoms of velvet:
  • Reduced or complete loss of appetite.
  • Heavy breathing, rubbing, flashing, head twitching, erratic swimming behavior (unfortunately velvet shares all these same symptoms with ich & gill flukes.)
  • Swimming into the flow of a powerhead (unique to velvet).
  • Acting reclusive (velvet causes fish to be sensitive to light).

The prazipro was finished on last Saturday. So it's been a complete week. Saturday I noticed one of the fish hiding wedged between my heater and the side of the tank. Didn't seem normal. When I came home from work, all fish were out and about.
I did put some fresh carbon in the QT yesterday as water had a slight yellow tint to it.

The wrasse had been acting funny for awhile. It was a leopard. I found him laying on top of his sand bed all the time. Sometimes laying on he floor of the tank. When I fed, he'd pop up and seem to become better. Eat a few pieces and then back to the behavior.

Don't know if you can see it or not but it looks like fin rot and white dots on the tail.
