I need help with my kole tang. His stomach area is bloated on both sides, he's a little aggressive (wasnt' before) and swims with a slight tilt. He's in a 90g with a few wrasses, clowns and cardinals. No signs of any diseases (fish are all qt'd).

I've had this happen (on multiple occasions) with my PBT. First thing I always do is feed her peeled boiled green peas (same kind you & I eat). I think the fiber contained in the peas helps to “push things out.” If dealing with a fish that refuses to eat, you can try Epsom salt (as a laxative) or an antibiotic such as Kanamycin (aka Seachem Kanaplex) that lists diarrhea as a possible side effect. You can dose Epsom salt (1 tablespoon per 5 gals) directly into your DT, as that is just Magnesium sulfate. However, antibiotics should only be used in a QT.

I fed some peas, and it turns out all my fish love them. I'm talking clowns, flame wrasses, flasher wrasse, yellow coris wrasse, Bangaii cardinals, even the mandarin was like hey what's this. The tang barely got any but I'll keep it up, along with live black worms.

Never seen anything like that! My tangs are morbidly obese from algae but that's as close I've seen

When to mix up fish meal: When was the last time you tried a different brand of food for your reef?
