Yup that is aiptasia. It is attached to that rockOff topic, is that Aiptasia on the sand bed ?

Agreed on the scutes.I'm not an expert on identifying these, but I associate Maximas with more pronounced scutes than that. Mine certainly has bigger scutes.
I'm wondering if that is a crocea, but consider this just a bump until one of the clam gurus shows up.
Im thinking maybe crocea also but I too am not a clam guru. It definitely looks more like my crocea than maxima. These are all smaller clams 2ish inches. It COULD be a maxima in bad condition as I see little growth and the scutes could have been eroded over time.
Either way the clam doesn’t look healthy

In my experience, a clam in this state will eventually just wither away and die.What should I do about it’s health? Do you think it will die?

Crocea and Maxima I keep on the rocks under 400+ par. I've only had this tank running 5-6 months so clams are not very old but I'm seeing signs of growth.Are all these on the rocks? My main question here is where should I place it. Rocks or sand?

I think all you can do is put it up high on your rockwork in a stable place and give it as much light as possible. My maxima is under nearly 400par as measured with a seneye. General wisdom is that is the lower end of the norm for Crocea and Maxima. More won't hurt it if you can manage higher levels.What should I do about it’s health? Do you think it will die?

Generally, Maxima and Crocea are placed on the rockwork. Some other clams prefer to be on the sand, but not the one you have.

I think it is in the brightest spot in the tank. The light is directly above it. It must be happy with its spot if it attached alreadyIt's a good sign from a health perspective.
