I have had my fish back in my aquarium for 3 months, after ich treatment, and fallow display.
How soon could ICK come back?
My reason for asking is that im seeing some faint spots on my spiny box puffer.
They are not showing up on a photo, but I can see them.

Yes its possible and nothing to do with sand. These fish are prone to ich. What treatment did you use and at what level?
 
How long was the fallow and at what temp, what quarantine was done on the fish
 

Just to clarify , that post is for freshwater ich, a different genus of parasite. It also doesn’t take into account the resting tomont stage. That can take at least 45 days to die out, leaving a tank fishless for 60 days is the best way to clear an infection.
