Sick plate

Well,
My orange plate, that survived to my crash 2 years ago, is now in very bad shape.
I want to know if it's gone......if still there's any hope or if there's something that I can do about it.
The water parameters are OK.....The only change is that I raised up Nitrates to 5ppm.
It's the only coral in bad shape in my tank.
I put it in the frag tank 2 weeks ago but I see no improvement in the condition untill now.
I have it for years, since it was a 1" golden disk and I'm really sad about it.

The one thing I've been told a few times about plates is they like stable alk but you have had it for years so you obviously know what you are doing
Dosent look to bad as in it's gonna die in a few days , wonder if you had a fish picking at it every now and than ...any peppermint shrimp ?
If it does die don't throw the skeleton out
They sometimes grow lots of little baby plates on it so you might lose this one but gain lots more in the future:)

Thanks for the answer....

My system is sps based, so my Alk is pretty stable.
I don't have peppermint shrimps (just because I have acans and IME, they tend to nip them).
I never see any fish nipping and, for now, It will stay in the frag tank where ther's no fish.
I am doing pretty much nothing untill now besides wait for some recovery.

Maybe a lugol's bath can help.

Did anything fall on it for a while? I've heard of pockets of sand or even shells or something irritating the tissues and causing them to do this.

I have a golden head sleeper goby that throws sand over EVERYTHING in the tank....
It's a possibility.
