You know exactly what you doing. This is not an issue of knowledge or skill, you have both. It's not an equipment issue either. I did not ask how you manage ALK/CA. A calcium reactor? Kalk? I think what I would do at this point is change salt. I keep coming back to instant ocean for some reason having tried many. Here is a thought! Many of us have a HUGE water container to store mixed water for changes; is it outside? Could it have become contaminated?

Is it an issue of feeding more? I feed my fish a lot and it might help the coral as well.

Still carbon dosing?
not since the reset, i having hard time keep the no3 up. before the reset, nopox make it impossible to have NO3. i was dosing 500ml KNO3 per week. i still test zero.

Cheato! That could be it! I run an algae scrubber (Oversized) and if I run it 24/7 it strips something out of the water that the corals don't like. I notice it 1st in my mushrooms, they become grumpy! So, I clean out the scrubber, change water and they get happy again.

I would cut the cheato size. At least think about it.

it was carx with kalk, after the issue i try to eliminate as many variable as possible. so carx is off line, only Kalk atm

i have a fishroom, both mixing tanks is in that room with locked door, so my kids wont go in there, and they know they cant go in there when i am not there.

6 X 1" cube of frozen food per day.
i got about 28 fish, between the frag tank and display.

Maybe lower the intensity of
Lights till Things recover a bit
if i cut any lower, the bottm of the tank will drop below 75. i do have a lot of shroom at the bottom of the tank.
when the whole thing started, i thought the light might be the problem too, so i lower a few, so far zero sign of recovery.

Any of us could be right. It could be the RO. It could be the Cheato. I would check impellers to make sure nothing is broken. Check magnets, etc. Do a few extra water changes just in case. But I'm out of ideas.

Poly filter always good thing when I experience mystery issues
poly filter catch heavy metal which the ICP rule out, that's my first thought to, i thought some metal or wear and tear from equipment. ICP rule that out too.

i did check all the pump and magnet, i have a tunze magnet did that nuke all my sps, almost identical to this, just slower than this, it take a few months and sps dying one after another. this time it's all the sps went at once within 2-3 weeks.
i have been doing daily 50g water change since i collected the water samples to send into ATI.

Instead of swapping out a $50 membrane your trying to convince yourself everything is fine because a 3rd party vendor test result said so

I would never trust a 3rd party over myself

if that's the case, what the point to have science, you might as well just do some voodoo and hope for the best.
if you cant trust icp, what make any test we did at home any good. since someone else make the test kits.

I would change salt in the event the mix is missing something important. I would review the flow to make sure nothing is being blasted directly. I would go over my lighting spectrum just to make sure what I need is there.

That is what started my mess a magnet on my hydor .
It does not take much. I'm waiting for a new pump as I type. The plastic in my return pump magnet is swelling with a hairline crack! Arhhh...
