This looks like scars from scratching which may be associated with flukes . If it is..... some signs you would see are rapid breathing, fish acting lethargic or swimming near the water surface, hiding in the corner of tank or behind rocks, loss of appetite, shaking its head, flashing/darting, develop clamped fins, , or scratching against objects. They may also exhibit what looks like yawning from gill irritation develop, cloudy eyes and loss of color.

It’s not scars. It’s getting worse. Imagine like Crustys from your eye but on the skin of the tang. It’s raised white stuff and it comes off when you net him

Pics fuzzy but appears as bacterial and would recommend isolation in a separate tank or container with air stone added and treated with Seachem Kanaplex

First thing I would do is remove him and put him into a QT Tank with fresh saltwater, airstone , and a heater. Could it be excess Mucus and silt and mulm sticking to him? Clearly sick because he's not moving and breathing hard.

Try amazon and see what they can do otherwise this will be an issue as treatment is essential in this case

What about melafix i know its api which is controversial. or Fritz Bactershield Bio-Adhesive Supplement
Melafix is horrible and Bactershield would be better than nothing

should i throw on a uv filter?

Atypical Head And Lateral Line Erosion shows up on this species like that. How long have these fish been in captivity, have they been exposed to carbon filtration?

no carbon and in my tank for 6 years

The length of time certainly is correct for it being HLLE. I’ve seen sudden onset, but that is rare, generally it develops over months.
