This looks like scars from scratching which may be associated with flukes . If it is..... some signs you would see are rapid breathing, fish acting lethargic or swimming near the water surface, hiding in the corner of tank or behind rocks, loss of appetite, shaking its head, flashing/darting, develop clamped fins, , or scratching against objects. They may also exhibit what looks like yawning from gill irritation develop, cloudy eyes and loss of color.

i moved him into qt thats why, currently swimming and eating like a pigFirst thing I would do is remove him and put him into a QT Tank with fresh saltwater, airstone , and a heater. Could it be excess Mucus and silt and mulm sticking to him? Clearly sick because he's not moving and breathing hard.

no carbon and in my tank for 6 yearsAtypical Head And Lateral Line Erosion shows up on this species like that. How long have these fish been in captivity, have they been exposed to carbon filtration?

this was over a 1 week spanThe length of time certainly is correct for it being HLLE. I’ve seen sudden onset, but that is rare, generally it develops over months.
