Toadstool Leather Help!!!

My leather which is quite large starting getting a whole in the center and loosing tissue. Now it just seems to be getting bigger.
Any ideas or what I should do thank you!
Parameters are all fine everything else is very happy.

Two things cause this:
Worms- which I doubt and would be visible , but still check

Propragating which I believe is the case. At some point, they literally tear themselves piece by piece and emit little frags of themselves for reproduction. Almost looks like someone took a scissors to it.
Provide medium flow and moderate light to it while it does its thing. Generally this lasts up to 2 weeks
