His poor eye. It looks like it has been injured and has swollen up... infection setting in as well. If you have a QT, it may be simpler to just remove him to there and treat in QT. Epsom salt for the swelling and antibiotics like Kanaplex or Furan2 for the infection.

Epsom Salt: Treats eye and other injuries. Popeye in one eye usually means injury, but popeye in both eyes usually denotes a bacterial infection requiring antibiotic treatment. Cloudy eye can be a bacterial infection or flukes.

How To Treat - 1 tablespoon per 5 gallons of aquarium water. Repeat only once, if necessary. Use only “USP” grade Epsom salt.

Pros - Reef safe, as Epsom salt is just magnesium sulfate.

Cons/Side Effects - The effects of excessive sulfate in a reef aquarium are not fully known.

Man I'm real sorry :( But it does look like there is damage at the top of the eye. Hopefully you can get it cleared up for him and have him back with his buddy in no time

I know how ya feel. I really love my bangaii cardinal as well. I've had him for almost 3 years now. That eye doesn't look good, and you might not be able to save the eye itself (though it's possible with quick treatment), but you can certainly save the fish. At the shortest it will take 10 days in QT with meds and lots of vitamin rich foods to heal him up, at most a couple weeks.

His eye was better yesterday somewhat. There were a few strands of white stuff hanging/ floating from his eye. Today it was back to looking really bad.[emoji21]. Since 48 hrs had gone by at 5pm, I did a 30g WC. I then added Furan-2 as the KanaPlex hadn't arrived, as I suspected:(. I dosed Furan-2 which showed an expiration date of 9/2016 as that is all I had available. I'm going to add some USP Epsom Salt here pretty soon...I hope it helps the poor guy. I cannot think that the eye will be saved the way it looks. I'll keep the faith though![emoji38]. This is day 2 that I haven't seen it eat. My fish are healthy so I hope that will help him hang in there!

Sorry to hear that[emoji17]. I added the Epsom Salt 3 hours ago... I hope it soothes it's eye. It will need to eat tomorrow so it doesn't get too weak. Need to keep it's immune system going!

I'm sorry sweetie. It can take time for the epsom salt to work on the swelling. Most of the time its just a few days though. I really think he's going to need antibiotics and needs to be moved to a QT for that.

Thanks. He has a part of the eye stuff hanging from it. I have a QT of course but I can't catch him. He knows where to go in the 180 so I and the other fish can't get to him. I won't do a WC until the new KanaPlex gets here. Will I need to add additional Epsom Salt after the WC? Thanks for your help!!

Looks like the swelling is going down so that's a plus. If it's still swollen after the water change, then you could add more epsom salt. I don't think it will be needed though. The antibiotics are going to be important at this point.

He ate shrimp this morning! Yay!! The piece hanging came off also. This is a positive sign! No if the medicine would just come! Here's him this morning, still hiding

Yes! Not many stores are selling the KanaPlex:9( Amazon still hasn't shipped it yet. So today I did the Furan2 again. Should have it tomorrow or Friday....finally. Amazon wanted over $50 for 1 day shipping...terrible and with my medical bills now, I just couldn't :9(

He not only ate shrimp this morning but he also ate my frozen food!! AND, he followed the Mandarin to the center of the tank! WooHoo!!

His eye looks the same today. I do not see any swelling. Tomorrow I will do a WC in hopes I get the meds. Thanks Melypr1985 for all the help and support!
