So i have lost 2 fish and 1 is sick right now but not showing any signs of the usual suspects.

Maybe ill start from the beginning since its a new tank.

Had a nano for 2 years and started a 90g in september 24
i transfered over 2 clowns and a royal gramma.
I start adding some fish to fill out the tank, Tomini tang, squamipinis anthias and a yasha goby.
All these fish are still doing great after months in the tank.

Then it starts,

One morning i find my royal gramma laying on its side, struggling its way around the tank unable to swim straight. other than this no other visible signs of disease.
I put him in an isolation box in the tank and he gets progressively worse for the next 2 days.
I decide to euthanize the fish cause its was a obvious goner and i didn't want it to suffer any longer.
I do some research but find nothing that fits, i guess i just got unlucky.

2 weeks later i get 2 bangaii's from my LFS and order a starry blenny.
They are in for a week and one morning i find the larger one of the two in a cave leaning up against the side, i was kinda worried and use my tweezers to see if he's responsive.
He starts flapping for his life but just like my gramma he cant swim straight.
I run for the isolation box and just when i want to pull him out he gets sucked into a powerhead and well... that was all he wrote.
Now i'm really bummed out.

3 days later i get the call my starry blenny is in, i pick him up from the LFS, acclimate and put him in the tank.
3 days pass and now its today, i get home from work and he is leaned up against the glass breathing rapidly, no visible signs of disease.
I get out the tweezer and check the response, he is not able to swim straight and cant keep himself up when "standing"
He's in the isolation box now just sitting there breathing fast.

Tank info:
90g with skimmer, UV and GAC

Parameters:
1.025
7.3 DKH
Po4 0.03
No3 8
Seachem ammonia alert reads safe
24.3c / 76f

Dosing:
ATI essentials+ 1+2 ( tiny amount, almost no coral in the tank )
Po4 ( to keep off 0 after dinos )
Mr holmes farleys ammonia recipe for keeping up No3

I don't have any pictures of the gramma or the Bangaii.
But this is the starry blenny.

I dont trust alert badge, an $8 device made too for freshwater to sustain $59, $00 or more in fish. I dont see mention of quarantining which is highly recommended especially with previous loss of livestock.
Are there any snails, shrimps, coral etc in tank? If so, how are they looking?
Were fish breathing normal when problems started or labored breathing?

I have a quarantine system planned but having my tank replaced because of a building error kinda messed up my planning.

1 blood shrimp in the tank, 3 turbos ,10-12 trochus. and 2 conches
i did lose a couple snails but nothing that worried me ( 2-3 )
the others all look good.

3 corals: frogspawn, stylophora and a toadstool.

As far as i can tell they were breathing normally before the problems started.

I would consider either velvet - since it's so quickly moving through the fish and killing them - and/or flukes. Though the alert badge is not perfect, if it showed 0 ammonia I would tend to believe it. If your ammonia were quite high it would/should affect all the fish, IME - but you can always re-test it.

Can you post a video of the remaining fish, under white light and close enough so that I can get a respiration rate count?

Anthias and bangaii.

Neither fish is breathing fast, so that rules out gill parasites. I can't explain why though, the Banggai is swimming with that jerky motion. Is there a strong water current where it is handing out?

yeah there is a bit of flow where he hangs out, if he's just chilling in the corner he swims less jerky.
All the fish in the tank at the moment (6) seem perfectly fine to my untrained eye, swimming, active, interacting, foraging and actively going for food.

So a follow up question,

What do i do now?
Do i set up a quarantine system and quarantine/medicate my next fish before adding it?

Or do i setup a quarantine, move all my fish over and go fallow on the display? This would not easy, only have a small Q tank, worried about agression and stress with putting 6 fish in a 8g tank, i might be able to get a 25g but that still seems small to contain and medicate 6 fish for 2+ months.

Trouble is, there is no clear diagnosis here....

Flukes are one possibility, given the fish losses spread over time, and at least the one fish taking a few days to die.

You can safely treat the tank for flukes. It might not be the issue, but it is the most likely, and the treatment won't harm your invertebrates. For Prazipro - you need to dose it to the actual volume of the tank (like 85% of its rated volume to discount rock displacement, etc.) and add extra aeration. After 8 days you do a 25% water change and dose it again.

I have a European formulary, but some of the products are no longer available due to changing laws in the EU:

Those shown to contain praziquantel include:

Dactymor, eSHA gdex, Fluke- Solve, Gyrodol Plus, Fluke P, and Tremazol.

You would need to know the amount of prazi in any of these products in order to calculate a standard dose of 2.5 mg/l
