Ich? Recommendations?

I have a hippotang and clownfish in my tank and just added a small watchman goby, I fed my fish (no spots) today and topped off the water then left until very late tonight. The water had evaporated almost a gallon in my 20.5g tank and now my fish have these white spots that I’m assuming is ich?
I adjusted the return pump to agitate the surface more and cleaned the filter sock in hopes to clean out the film at the top of the tank. There’s white spots in the film and bubbles that aren’t normally there. I already readded the water before I noticed the spots on my fish. The clownfish has maybe 4 spots on the top of a fin - I couldn’t get any pictures of the clownfish.
The goby I haven’t seen since I added him.

Do you have any tips to help me keep my fish alive?
I can’t quarantine the fish unfortunately.
Should I go and get an ich medicine?

These are mucus cones often from a skin irritation and at times are mixed in with ich and Often you will notice other occupants show no signs. You can wait and see if it worsens or recommended is to remove all fish and treat them as well using coppersafe or copper power for a full 30 days at 2.25ppm which they respond to well and add an air stone and monitor copper level with a reliable Copper Test kit such as Hanna Brand- No API brand. Also monitor Ammonia levels while in quarantine with a reliable test kit and add aeration during treatment using an air stone.

This could be ich, could just be mucus cones, or it can be a mix of both.

There won’t be any ich medication that will work with any corals or invertebrates in the tank….do you have any invertebrates?

As a side note - you mentioned a film on the water’s surface. It looks like your water level is above the level of the surface skimmer slots. Adjusting that will skim the water’s surface better.

I do have 2 peppermint shrimp and 4 hermitcrabs - this morning most of the spots seem to be gone. There’s now only a handful of spots on my tang that you can only see if the light hits it perfectly. Should the rest of these go away on their own?

Part of the life cycle of ich is that the white spots (trophonts) drop off the fish to reproduce. Early on in the infection, they are all in sync so it looks like the fish is improving, but then the spots return in greater numbers. Mucus plugs tend to stay in the same location for days at a time, then just fade away.
