Illness ID Help! Is it Ich?

Hi all! I posted in another thread but remixed this one might be more appropriate.

Hoping for a little help with what could be going on with my tank. I recently lost three fish to what seemed to be ich - royal gramma, damsel & sleeper goby. I also have a clown and a lawnmower blenny that appeared normal when they got sick. Started with these three with some noticeable white spots. I started a QT tank and treated with Nox Ich, but unfortunately none of them made it - RG/Damsel First and a few days later the goby.

Then the following week I noticed some obvious irritation with my clown and blenny. The clown does something that seemes like a twitch. It’s not constant but does happen several times daily. The blenny started hanging in the corner and occasionally biting at his tail fin. But agin nothing noticeable on them.

So in a new QT tank they went with Nox Ich but I’m not certain that’s what it is because they’ve been treated for the past three days with no real difference in the behaviors.

I cannot see anything on either of them and up until the QT the clown was eating, but now is not. Blenny is still showing interest thankfully but I want to save both of these little guys. I’m so afraid to do a FW dip because of the stress it’ll cause. I do have Prazipro on hand but have not used it. With all the info out there and the less obvious diagnosis here, it’s hard to make an educated guess on how to treat.

Any tips or suggestions would be wonderful.

Why did you move the clown and blenny if they were fine? My blue hippo had crypto two weeks ago in my display. I just kept feeding. He got better, no other fish got sick, no issues, no harsh chemicals. The stress from moving your fish and making changes is what’s killing them, not crypto

I moved the other two because they started to behave as oddly. Clown - twitching/some irratic swimming. Blenny - more lethargic and staying in one corner. Both abnormal for these guys and since I just lost some others that did have white spots, I assumed what o was observing was some early signs of illness. I didn’t move them until I was certain something wasn’t right. I honestly was very reluctant to do so but assumed if Ich (or worse!) was in the tank it was the best shot they had.

Sorry about the loss of your fish. The symptoms you are describing are very common for flukes. The danger is a secondary bacterial infection kicking in because of the damage the flukes are doing to your fish's immune system. Freshwater dips while looking awful are usually fine with fish. Thanks, never easy to watch them crash. I was kinda thinking it might have been flukes too but again so hard to tell. I’ll give a look through the FW dip link and give it a shot. Once the FW dip is done I assume keep them in QT? Wish me luck!

Just keep in mind the FW dip only remove flukes on the skin of the fish, not the flukes that may be in the gills. So you may or may not see a lot of the little sesame seed size bodies of the flukes after the FW dip.

Unfortunately, I lost the blenny :( but the clown still remains. So this morning I did a water change on the QT tank and dropped the salinity to 1.018 and added Prazipro. When I got home she seemed to be breathing rapidly. I did a FW dip on her. She took it like a champ, but still seems to be a bit stressed once back in the QT tank. One small white thing did come off of her. Here are some photos post FW dip for what it’s worth. I’ve heard some other users mention velvet and to treat with cupramine. Do you think this looks to be the case? I’m starting to feel like we might be fighting a losing battle I figure we might as well keep trying if it’s not too stressful.
