Lost my yellow tang today

I hadn’t seen him today and Just found him floating behind a power head. Had him over two years and I don’t have any disease in my tank.
I thought maybe my kole tang may have went nuts on him but No sign of injury.
I did a water change yesterday and siphoned some sand but that’s nothing unusual.
Rest of my fish are eating and seem okay.

I’m sorry!

This is my theory and there are MANY that will disagree... but I never do gravel cleaning when I change water. I believe it releases bad stuff into the water column. The top layer gets moved around a little by fish, crabs, etc but I never do gravel washes....
