Is it newly added to the tank? Typical of a stress bacterial infection.

Looks like ich insertion points with the thick slime coat's reaction around it perhaps. What other fish are in the tank, and do they show any spots or symptoms? Any twitching? shaking head? scratching? flashing? swimming in to powerheads? spots? "dusting", cloudy eyes, red marks, wounds?

Hi so its been another month. The black spots come and change evey every few days. No other fish are affected. The foxface's eyes look slightly cloudy under blue lights. No flashing scratching heavy breathing etc, eats like a pig. I have an anmeone but he would have to like roll in it daily to for these to be stings and hes never near it. Theres are angels, tangs, chromis, cardinals, gobies, clowns, royal gramma, mandarin, sailfin blennies in the tank, as well as shrimp snails tux and pincushion urchins, and nems. Focus and metroplex in the food as well as garlic and selcon havent helped. Ill try prazipro in a couple days. Any other ideas? Its so weird, the only thing that looks kind of close is black ich but idk if its that.

The spots are black because if I remember correctly the Foxface has a Anti-Parasitic slime coat. If his eye is getting cloudy I would FW dip to check for flukes.

Bump.

I have the same issue with my Magnificent foxface except I’ve not changed anything (I added one coral last week then my UV wasn’t sterilising and my tang is showing ich). My foxface is the oldest fish in the tank at 4.5 years, then my tang is close by at over 3 years (added in March 2020). I’d rather not lose these two however currently I am just power feeding plus I added a cleaner Wrasse to help with my Cleaner Shrimp. I just wonder if the OP may have any updates.

I believe those are sting or injury. Maybe there are parasites that bore into the skin. That is why you see central spot with what seem to be leaking from the skin, and dark area around it.
Second thing that is wrong with the Fox face is that it is very think. The muscle at the dorsal area should be think and bulging out rather than concave in like this fish. I would increase feeding, if this does not correct the problem, he needs to be treated for worms.
