the toadstool is not dead at all, its just slumped over. Corals need lights, flow and nutrients to survive, please tell us about your lights flow and parameters

I’m running a nicrew 65 watt and I’m trying to give indirect flow, parameters : temp 78 no ammonia no nitrate like five ppm nitrate and calcium-550 mg/l Phosphate is something around that what else do I test?

What are the water parameters? What test kits are you using? What is the water temp? What lights are you running?

I’ve moved mine between several tanks now, and it does seem to have a reaction similar to this, I haven’t had it all the way down like that but I think it’s just adjusting. These are very hardy corals, however when they get moved or shifted their foot needs to adjust, it doesn’t move around per se but it should adjust over a day or two. Then pop itself back up

I just noticed the mushroom as well. If your temps are okay, and salinity how far from the water level are your lights and what level are you running them at?
Flow isn’t too high? Should be rather gentle on them.

btw, in a week (and occasionally in the future) it may basically shed its skin on top. Nothing to worry about, it's normal. Also, what type of mushroom is that? Ricordia can be tough to keep alive in a new tank.

Take some video, post it on youtube, and then post the link here so we can see what flow looks like.

If nitrates are zero (your statement was confusing), that's not good, you need nitrates to feed the corals.

Toadstools like a good medium+ flow. So get it in some more flow.... it'll probably perk right up.
