One of the worst mistakes I ever made was touching a 250 watt mh bulb seconds after turning it off.

Ouch! That had to hurt.

Me too touched a 400 watt metal halide with my arm reaching in the water, WOW did that hurt huge burn, skin was stuck to the lamp...

My old 450g tank was indoors an so was my ro unit. Instead of using john guest fittings I had the old school ferrel and nut fittings, didn't tighten one properly and checked it twice before going to bed,but still woke up to 1.5" of water over the whole living room-I hoped it was a nightmare but it wasn't

Worst mistake had to be in my first months of reefkeeping... Bought many freashly transshipped fish and added them to the new tank.. with in days they all were dead and dying from ICH.. so I bought some more fish..... 30 days later they were also dead so I got really upset with LFS and he came out and added a "Miricle cure" to the water that was going to kill the ICH... so the next week I bought more fish and they all died :( .. LOL just imagine I was about to give up

I got careless with Joesjuice or kalk and a glob of it went into the suction hole on my $350 Blue Teardrop Maxima. Needless to say about 2 days later it started to gape and 3 days later it was dead.......I tried to get the crap out of it but it kept closing up on me. I did the same thing to another $250 Maxima which also dies a few days later.......I could have cried after each one.....
