Not sure if something got the clam or what happened. Had it a year now and it has always seemed very happy. Could it pull through or is this a death blow?

Oh no. Tissue retraction. You can try a freshwater bath to eliminate any irritation.
Salinity and ph is important. Salinity 1.024=1.025, and ph of 8.1-8.3 and temp not to exceed 80 deg (pteferable is 76-78) and no strong current.
Any angels or butterflies in tank ? Also, with a flashlight, look into clam for presence of any crabs or tulip snails.

What about that anemone close by. Could it be a problem?

I do have a flame angel in the tank and had to relocate the clam about a month ago away from bta's. I did recently have to do a large water change that was slightly over 1.025 where my tank normally is. Once I get the day off I'm gonna get a plan together to get the bta's out. I have over 30 now.
