I have had my 65-gallon tank set for a little under two years, and I thought I was getting my setup stabilized but I have encountered numerous problems in the past few months. I cannot seem to keep any LPS corals alive in my tank, even though my parameters seem to be good. I have had two gonis, two hydnophora, a hammer, and a chalice all die within the past three months. All corals seem to be experiencing polyp bail-out, as the tissues just pop out of the skeleton without much decay. I've been testing the waters pretty consistently, and my parameters are bouncing around between tests, but I've been dosing to try and keep the levels more stable. After holding out for a while, my torch has now started not to look well. I've dipped it just to make sure there are not any parasites, but one of the heads has completely shriveled up and today the other has started to deflate a bit. Any advice is appreciated, I'm trying to develop a better testing/dosing routine but I've felt really defeated the past couple of months with all the loss.

Salinity is consistently at 1.026
*I know the nitrates are high, I was down a protein skimmer for a bit but just recently installed a new one as well as fresh carbon.
