Help. Torches are dying

I am not sure why, but my torches are dying one by one. I suspect that it is brown jelly disease. I took one out to try dip but 1 head just disintegrated when I was stirring. My salinity - 1.025, alk at 8.5ish, nitrates at 5ppm.

Tank looks fairly new. How old is the setup? How long have torches been in there? What is your lighting? Any other info you can give?

Where are your phosphates? I'm assuming your parameters are within normal range as well.
 

Looking back to your reefing roots: Did you start with Instant Ocean salt?
