So I've noticed for the last couple of days that my open brain hasn't been as open as it normally gets during the light cycle. When I looked today, I can see part of the skeleton centered around one of the mouths, as well as some of the tissue just falling off the skeleton.

Should I leave it alone, or frag it in half, hoping that the healthy tissue on one side of the skeleton will not be effected by whatever is damaging the other half?

Water parameters are all normal...I added a chalice to the tank about 2 weeks ago, but put it about 6-8 inches away and down stream from the open brain, so I find it unlikely that this is causing the issue either. Any help or advice would be appreciated.

None. Just a perc, a watchman goby, and a black cardinal. It seems like one of its mouths is gaping.
