Torch coral losing tentacles

I’ve tried FB and looking online but can’t seem to find my exact issue…my torch’s and elegance are long time happy residents. Up until the last 2…3…weeks. I was bringing my tank back up from 0.0 nitrate and 0.02 phosphate (now 6.0 and .04)
However over the last few weeks I’ve noticed my torch’s and elegances tentacles have become twist and in the case of my torches they have lost most/all of their tentacles. other than 1 torch that won’t open up at all, all the torches open up just without tentacles. My numbers are stable for the most part. I am also running a carbon and gfo reactor.

In one of your photos, it looks like brown jelly on one of the heads. That is not good for torches. If so, you'll have to do a dip.
 
Specifically something like ciproflaxin. I'll let other disease experts chime in as I've only read about this on R2R and haven't had to deal with it (knock on wood).

I haven’t seen any brown jelly, but I’ll look tomorrow and the lights come on
 
This can happen for a number of reasons. Swing in salinity or any swing in the big 3 or in po4 and n03 will cause this.


Tank maturing can cause issues with corals aswell.

Salinity is 35-36

since posting this I have learned my cal and mag are out of balance and I’m working on that but don’t believe it to be the cause of this. Just sucks watching things melt away
 
Cal and Mag stability are super important but I would say its just part of a larger problem with bailouts.

A little over a year and a half, and everything has been solid prior to this. The rest of the tank is happy

The period of time you bottomed out nutrients could have a negative impact on their health. Have you done a recent ICP to check everything?
 
I am not a fish medic or invertebrate vet, but as MD I like to diagnose disease first and then throw multiple broad spectrum antibiotics if indicated. You don't want treatment to do more harm than good.It seems as your torch was deteriorating in last few weeks, that would be less likely due to BJD and more due to some shift in conditions.
