a cleaner wrasse and mandarin in a six month old tank is a no no as well. Carpet nems can and do eat small fish so be aware of that. First get some test kits and test for ammonia, nitrite, and nitrate.

The plastic is just a container thats where the anemone was when he suddenly moved lolz

Are you able to return any of the fish you purchased to your LFS? I would return all fish except the pair of clowns. Keep in mind, every fish in your tank, with the possible exception of your clowns, were pulled from the ocean for your enjoyment. You owe it to them to do the best thing for them, so return them to your store and start fresh. It looks like you are having a bit of an algae bloom as well, expect that to continue until you remove the majority of your fish.
Once you have returned the majority of your livestock, be thoughtful about what you want to put in your tank. Being 40g, you will have to keep the max size of the fish on the smaller size. There are plenty of smaller fish that have tons of personality and color. Do your research first and then make your choice/purchase. Good luck!

If you have a good clean up crew (snails, crabs, shrimp, etc), they can take care of a decent sized fish in hours.

I hope this help last week we got rid of our big carpet anemone (toke it to LFS). Because my clown fishes start disappearing and no dead fish were found when we look in the tank. Because when they dies you see them around.

By the time I got home from work the most of them already died luckily my blue tang 2 clowns 1 dont really know the name and all my inverts are alive a rushly took them out and did a replenish on the water and now waiting game for the cycle
My corals seems to be ok but my giant carpet anemone seems to be in really bad conditon
Took all the lr in it and clean all the algaes green and violet/maroon or something color algae and suddenly some hairy worm fell of i mean big hairy worm like 5-6 pcs dont know what kind of inverts they are but its creepy so I threw them out hopefully I did the right thing

If it was exactly like that then it is bad. If it was similar it could have been harmless. They are bristle worms. Some kinds are bad some are not. I would return the live stock (fish inverts and coral) and then let the tank properly cycle. It usually takes a month with just live rock and a little bit of food. Then add one or two critters then wait another few weeks.

The best thing is patients with dealing with saltwater. I researched for over 6 months before buying my tank and I do extensive research before buying livestock. I've only been in the Hobby for a year and a half and I feel like it'll be at least 20 years before I can consider myself a novice with all the stuff that has to be learned. Don't get discouraged with your tank our pass decisions, it takes time and patience. Only add 1 fish after cycle and I always waited at least a month before adding another. Every fish you add, it adds a additional bioload to the tank and you've got to give it time to catch back up. There's a certain limit that the tank can handle, when you purchase a fish, look it up and find the adult size of that fish and base your system on that, that on actual size. They might be babies when you first get them, but they're going to get big. I see a lot of people who buy fish that isn't right for their system with plans of getting a bigger tank later down the road, don't make this mistake either. Only buy for your current setup, if you ever upgrade, buy the ones that is right for that system then. Good luck with your tank and remember ask questions and do research, don't trust the fish store, they mainly want your money.

You probably had a fireworm. It's a type of bristleworm that is very predatory. You also seem to have ich.
Starting over is your best option. Remove all animals for 6 weeks, and research during that time. Do you have an aquarium water test kit?
